My husband and I
are both 40 years old and had pretty much given up on getting
pregnant, or at least I had. We had been trying for 18-24 months.
I had been to a fertility specialist and had the whole battery of
tests run. Luckily there were none of those scary genetic diseases
that posed to be a problem, but I did turn out to have stage two
endometriosis. I had a laparoscopy in April of 2007 (at 39) which
was conducted without any complications, although I can’t say that I
particularly enjoyed my first experience with general anesthesia.
My FSH level was 5.6 at the time. While I really liked our doctor,
and I appreciated his directness, the prognosis he had for us was
less than encouraging. He said that before the surgery he would
have placed my chances for conceiving each month in the 1 to 2
percentile range. That was quite a shock. The endometriosis had
just been too much of an obstacle to overcome. However, even after
the surgery he gave us odds in the 8 – 10% per month range. Those
numbers rang in my ears for hours after the appointment.
I tried to look
at this as positively as possible. 8-10% is a dramatic improvement
over 1-2%, but it still didn’t leave us with much to cheer about.
After all, the reverse side of that estimate of course translates to
a 90 – 92% chance each month of NOT getting pregnant. His
recommendation at that point was to proceed with THE SHOTS. I had
switched jobs about two years before all of this, and went to work
for a company that covered fertility treatments to one that wouldn’t
cover a dime. So, we were faced with a cost of several thousand
dollars a cycle to give those shots a try. Even with the shots, he
estimated that our probability of getting pregnant was in the 10-15%
range each cycle. I wasn’t encouraged. He said we would probably
try that for three cycles and if we weren’t successful, we’d move on
to IVF.
As if THE SHOTS
weren’t expensive enough on their own, IVF was looking to run in the
$10,000+ per cycle range. If we did that with my own eggs, he said
we’d be looking at a 25% potential success rate of conceiving. And,
of course, conceiving (particularly at my age) is only one part of
the battle to have a happy and healthy baby. So, we thought about
it. My stepmother had been through the whole IVF thing for three
cycles several years back. She had conceived, only to miscarry at
about five or six weeks which had to be followed by a D&C. It was a
terrible emotional rollercoaster for them. I think this was one of
the biggest factors in our decision not to pursue “conventional”
fertility treatment beyond the surgery for the endometriosis. With
just my normal everyday hormones those days leading up to my
menstrual cycle were stressful enough. Each month I would try not
to get my hopes up, but no matter what I did, each month when my
period arrived it was a bigger and bigger disappointment. I could
only imagine what all of that would be like if I were stuck up with
more hormones than a prize hog at the Iowa State Fair (I went to
high school there).
The whole
fertility issue spiraled down to a true low in January where I spent
an entire night crying. I’ve never done such a thing in my entire
life. It went on for hours. This really impacted my husband.
Feeling a little helpless (his swimmers were fine by the way, so he
was already doing his part in that area) he started doing some
pretty extensive research on his own. He got books, he Googled, he
read and he read. He became convinced that acupuncture could help.
I wasn’t particularly optimistic.
He told me that
some of the best acupuncturists in the country were right here in
Houston. I still wasn’t sold. He told me that acupuncture could
help jump start and rebalance my female hormones to make conception
much more likely. I didn’t really want to hear it. I had used up
all my energy during the previous year and a half or more of monthly
disappointments. I wasn’t really interested in investing more
energy in what seemed to be a hopeless direction. I was pretty
ready to start investigating adoption. I figured, if we were going
to throw probably 20 something thousand dollars at this whole baby
thing, I wanted better than 25% odds of coming away with a son or
daughter that I was going to get with IVF. Somehow, he managed to
convince me to try the acupuncture. I don’t really remember how. I
think it had something to do with, “I can’t take watching you like
this anymore so you really need to try this idea I have so we can be
sure we’ve exhausted all our avenues before we give up.” As I
recall I told him that if he would pick up the phone and make the
appointment for me, I would go. He did, and I went.
My first
appointment with Jannie Chiang was on January 8, 2008. She asked
questions during my intake interview that somehow made me more
confident in the process. For one thing, she asked me a lot about
what my doctors had tested and treated me for, the results, etc. I
really appreciated the acknowledgement of western medicine by a
specialist in eastern medicine. I’ve always thought both would be
most effective if they took a page out of each other’s books. I
stated seeing Jannie twice a week and popping her prescribed herb
pills and tea like crazy. I kid you not. I had three or four
different herbs of which I took 5-10 tablets each three times a
day. At one point it added up to 60 tablets a day. There was also
tea. She explained that the herbs were to help raise my
temperature. She wasn’t pleased at all with the basal temperature
chart I brought in with each visit. “Way too low!!, Way too low!!”
So we changed from tea to powder, “Dragon Herbs” specifically, to
help heat me up.
This went on for
a few weeks. While I wasn’t sure if I was becoming more fertile, I
was feeling more relaxed than at probably any point in my adult
life. Some times, it almost felt a little like I was on drugs (then
again, I was taking an average of 45 pills a day….). I was used to
going through the day like a stress ball. Stress is generally the
only thing that helps me get from the beginning to the end of the
day. So I figured at the least, maybe I was adding a couple of
years to my life and my cardiac muscles were thanking me. I was
also hoping that the reduction in stress might make conception more
likely. Everyone has heard those stories about the couples who
magically end up pregnant right after they have adopted a kid. I’ve
always thought it was the fact that they stopped stressing out about
it every minute of every day that made it happen. In March, just
about two and a half months or so after starting the acupuncture
treatments, I took one of those early results pregnancy tests about
three days before my menstrual cycle was slated to start.
Negative. I took another one the day I was supposed to start.
Negative. Nada. Zippo. I had some mild cramping, but instead of
lasting for several hours followed by the start of my period, it
just went away after less than 15 minutes. I was really cranky, and
completely and utterly exhausted. I was also considering giving up
the bi-weekly visits to the acupuncture clinic. I had sort of
decided I’d give it one more cycle, maybe two, and then I would just
quit kidding myself. I had some friends who had friends who had
recently adopted, and I asked for their contact information.
Another two or
three days went by and I still hadn’t started my period. Then I
realized something incredibly stupid that made my mouth drop open.
Those two pregnancy tests I had taken weren’t pregnancy tests at
all. I was cleaning out the bathroom closet and realized that I had
been out of pregnancy tests and like an idiot had pulled out and
used ovulation tests instead. If you don’t pay attention to the
foil wrappers they look just alike! Of course those little suckers
were negative. Even if I wasn’t pregnant, my ovulation cycle was
well over a week away. So I made yet another trip to the drugstore
and purchased yet another box of pregnancy tests wishing I had
bought stock in that company two years ago. I made it home and just
stared at the box for a little while. Finally, I got up the nerve
to take the test, and not one but two lines appeared. Did I jump
for joy? No. I did what probably every woman has done when she
sees those two little lines whether she is hoping for them or not, I
got out another stick and somehow managed to summon up some more pee
and did it again. Yep. Still two lines. I sat there for quite
some time. Not feeling much other than …….hmmm…..two
lines…..pregnant……okay…… I was in a bit of a fog.
Needless to say,
we are thrilled even if it isn’t entirely real yet. I’m about to
kick off week seven, so there is still a long way to go. My husband
has taken to reminding me periodically how right he was about how
the acupuncture could help us. My mother chalks it up to prayer.
Personally, I’m believer that it took a bit of both. I also think
that without the initial surgery, that Jannie would have been
unlikely to help us successfully conceive. I think it took a
combination of eastern and western medicine, and faith. For a few
weeks, Jannie and my husband were the only ones who knew. I was
almost as excited to tell her as I was him. There was a pretty good
round of hugging following my little announcement too her! Not only
has she helped us achieve what I thought was impossible, I have come
to really enjoy our little chit chats while she is loading me up
with needles twice a week. A genuinely kind soul with a sharp
mind. Someone pretty nice to have on your side. Now, if she can
help us figure out how to deal with having a teenager around the
house while we are in our 50s………

Recent studies have shown that acupuncture definitely increases the
rate of pregnancy in women undergoing in vitro fertilization (IVF).
In 2002 a German study showed that a success rate of 42.5 % , 34
women out of 80 got pregnant who had IVF and acupuncture. Only 26%
(21 women) of the 80 women became pregnant with IVF only.
Another American study showed that 51% of the women out of 114
received IVF and acupuncture became pregnant, 36% without
acupuncture. Also with acupuncture only 8% of the women miscarried,
but 20% miscarried with IVF only.

Infertility theory in
the perspective of Chinese medicine
The
percentage of infertility is increasing a lot more through the years
from environmental factors, diet, stress, medicine, etc. Most
commonly seen in the female between the ages of 25 to 35 years old.
Infertility can be divided into two different types:
1.
Primary infertility - A couple with proper age and normal sexual
life, no birth control preparation, never had history of pregnancy,
but could not get pregnant after trying for at least two years.
2.
Secondary infertility – A couple with proper age and normal sexual
life, also had history of pregnancy, without birth control
procedures, but no pregnancy after trying for two years.
Absolute infertility in either husband or wife cannot be corrected
by any efforts. Usually it is a genetic problem and cannot be
changed in any way, so we do not discuss it here.
Infertility caused by compatibility issues is temporary and it can
be corrected by medicine or other efforts.
Etiology and Pathogenesis
The etiology and pathogenesis include two main reasons – excess and
deficiency.
Deficiency mostly is from insufficiency of blood, Qi, Yin and Yang
of kidney. Excess mostly is from the liver Qi and phlegm
stagnation.
1.
Kidney Qi deficiency: Due to congenital kidney Qi deficiency, abused
sexual life, chronic diseases or frequent miscarriages or abortion,
causes the impairment of kidney Qi, failing to properly nourish the
Ren and
Chong meridians, resulting in coldness and
lack of moisture in the uterus, leading to infertility. Ren
and Chong meridians are the most important
meridians to the reproductive organs. These two meridians
make a complete transportation of body essence, Qi and blood in the
center of the body to nourish the reproductive organs.
2.
Liver Qi stagnation: In Chinese medicine, chronic emotional stress
impairs the liver function. Emotional stress such as depression,
anxiety, anger, etc obstructs the liver blood and cause the liver Qi
stagnation. Due to the stagnation of liver Qi, the function of
liver to store blood is affected. Insufficient blood fails to
nourish the Ren and Chong channels properly. The uterus then is
short of replenishment from Ren and Chong meridians, infertility is
the consequence.
3.
Phlegm dampness: mainly due to a bad diet. Consumption of fatty
foods and other bad dietary habits damage the function of the spleen
and stomach. So the functions of transformation and transportation
by the spleen and stomach are impaired. The nourishment of Qi and
blood in the middle jiao gets slowed down and the vital Qi and
defensive Qi in the middle jiao also become weak. Therefore the
pathogenic factors easily invade the middle and lower jiao, where
the uterus is located.
Since the functions of the spleen and stomach are damaged, then the
function of water and moisture transformation from the spleen
impaired, the damp phlegm is produced more and causes the
obstruction of Ren and Du meridians. Without enough essence flows
from Ren and Chong channels in addition to the invasion of
pathogenic factors, the uterus is then disabled.
4.
Blood stasis:
1. Due to chronic imbalance conditions, such as congenital
blood deficiency, or deficiency of the spleen and stomach functions,
prolonged diseases, or invasion of pathogenic factors. All these
cause the disharmony of Qi, blood
and body fluid,
then “deficient heat” is generated. The
deficient heat stagnates the flow of Qi and blood, and then produces
blood stasis.
2. Due to improper health care after delivery or during
menstruation. Such as consumption of a lot of cold drinks or cold
food, or being careless about the weather change, or not getting
enough rest, etc, causes Qi and blood stagnation and subsequent
failure to warm the uterus. Therefore the damp heat pathogenesis
easily invades and produces blood stasis.
Diagnosis of infertility
The diagnosis of infertility is based on the analysis of the cause.
According to the differential of the syndrome to regulate and
nourish the productive organs.
1.
Kidney Yang deficiency: because of kidney Yang deficiency, the
nourishment of the Chong and Ren meridians are greatly reduced. The
reproductive organs could not get enough essence from Chong and Ren
meridians, and the uterus functionality is reduced.
Kidney
deficiency symptoms manifest on the lower back and bones. With lack
of circulation of kidney Yang, the loin and knees start to get weak
and painful. Since the Chong and Ren channels are deficient, which
fail to tonify the bladder and lower jiao, urination and bowel
movements are also then affected.
Symptoms: infertility long after marriage, delayed menstruation with
light coloring and amount, or no menstruation, dark complexion, pain
and coldness in loin and knees, no sexual desire, loose stool, clear
and profuse urination. Pale tongue with thin and white coating.
Deep and thready pulse.
2.
Kidney Yin deficiency: Due to kidney Yin deficiency, Blood, Qi and
body essence are reduced. When blood circulation is less, this then
affects the liver function to store the blood. As the liver blood
is reduced, the five sense organs and the head are affected. The
whole body’s nourishment is not enough; therefore muscle loss
occurs, the menstruation becomes light, there is a feeling of no
energy, and other deficient symptoms occur.
Symptoms: normal or
shortened
menstrual cycle with light amount and no blood clots. Weight loss,
fatigue, dizziness, blurred vision, tinnitus, hearing loss, and weak
lower body, a reddish tongue, with little or no fuzzy coating, deep
and thready pulse.
3.
Liver Qi stagnation: Due to Liver Qi stagnation, the function of the
liver is impaired such as storage of the blood and transportation of
the Qi and blood from the upper body to the lower body. Since there
is a shortage of blood supply and nourishment, the uterus becomes
weak.
Symptoms: Irregular menstrual cycle with light amount, darker color,
with some clots. During the monthly period, the patient feels the
menstruation pain. Before the monthly period, patient feels
swelling and pain in the breasts, plus emotional stress such as
depression, anxiety or irritability. Tongue color is normal or red
with taut pulse.
4.
Phlegm dampness: In Chinese medicine, being overweight causes the
damp phlegm in the body. The phlegm blocks the Qi and blood
circulation, and obstructs the nourishment replenishment of
meridians and organs. It leads to the poor circulation of Qi and
blood in the whole body and reproductive organs.
Symptoms: obesity or overweight. Delayed menstrual cycle with sticky
discharge or even no menstruation. Pale complexion, palpitation,
dizziness, nausea, and a feeling of stuffiness in the chest. Tongue
coating is white and greasy. Slippery pulse.
5.
Blood stasis: Due to the blood stasis blockage, the replenishment
of reproductive organs is insufficient. The stasis associated with
the menstrual cycle causes the blood clots and menstrual pain.
Symptoms: Menstruation is painful associated with clots and pain in
the lower abdomen. The tongue color is purple with dark purple
spots. Taut and thready pulse.

1. Kidney Yang deficiency
Treatment Method: Tonify the kidney yang, replenish Qi and blood,

2. Kidney Yin deficiency
Treatment Method: Nourish the kidney yin and increase the body
